The type of personal information I collect
I currently collect and process the following information:
- Personal identifiers, contacts and characteristics (for example, name, telephone number and email address)
- Treatment related details, like area of concern, previous injuries or operations – this is by filling in a form, which you’ll also have to sign to confirm this is all correct, that you’re happy to receive the treatment from me and that you’ve informed me of any previous or medical conditions
- I’ll also ask you to sign a form to say you’re happy for me to store this information. This is necessary in order to provide future treatments and keep record of progress.
How I get the personal information and why I have it
Your personal information is provided to me directly by you, via phone, email or right before the treatment and for one of the following reasons:
- To send you a booking confirmation, a reminder and to contact you for your upcoming treatment and any changes
- For treatment details – so I can treat you properly and effectively – now and in the future
I won’t share this information with any other organisations or individuals.
Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing and keeping this information is: I have a contractual obligation and a legal obligation.
How I store your personal information
I’ll hold your personal and sensitive data for seven years. This is required for insurance purposes. The paper forms are safely stored in a locked storage at my business address Wellington Ave, Christchurch, BH23 4HL. I make sure that all digital data (phone numbers and email addresses) are password protected. If you’re under the age of 18 at the time of treatment, your data will be destroyed 7 years after you turn 18.
I will not pass on your personal information to any third party, it is purely kept for use by Highcliffe Fitness and Massage Therapy only.
You have the right to request access to any personal data held by me and the right to complain to the ICO (Information Commissioners Office) if you think there is a problem with the way I am handling your data. The ICO’s helpline is 0303 123 1113.
Your data protection rights
Under data protection law, you have rights including:
Your right of access – You have the right to ask me for copies of your personal information.
Your right to rectification – You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.

Your right to restriction of processing – You have the right to ask me to restrict the processing of your personal information in certain circumstances.
Your right to object to processing – You have the the right to object to the processing of your personal information in certain circumstances.
Your right to data portability – You have the right to ask that I transfer the personal information you gave me to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
